YOURhealth Coaches

Our YOURhealth Coaches are on hand to support patients registered with any Cygnet Primary Care Network to make positive changes towards their health and wellbeing.

The YOURhealth service delivered by Humber Teaching NHS Foundation Trust have partnered up with Cygnet Primary Care Network to provide personal support and motivation with changing behaviours and reducing the risk of long term health conditions to local residents.

Patients registered with any of Cygnet PCN’s 5 GP surgeries can self-refer into the service to receive free one-to-one support which helps you set achievable goals, overcome barriers, and develop a personal plan to make positive changes to your health and wellbeing for the long term.

The new YOURhealth Coaching team will spend up to an hour with you at every meeting, and can meet with you weekly or fortnightly, in a way that works around your life.

  • Weight management
  • Alcohol support
  • Sleep Hygiene
  • NHS Health Checks
  • Group support such as YOURhealth & Menopause
  • and much more

The team will support you for up to 8 sessions over a 12 week period, allowing time for engagement, commitment, personal reflection and behaviour change. The aim being to provide you with the skills and tools to engage in long term sustainable personal change and generally feeling happier and healthier.

Alongside our 1-1 support is our new YOURhealth & Menopause group support programme.

Our YOURhealth & Menopause group support is a six week educational programme looking at all aspects of womens health & wellbeing supporting you through your menopause journey. This includes looking at ways to help you to manage your symptoms, either alongside medication or without through healthy eating, sleep hygiene, relationships and more.

Resources

We want to be able to support you to move from surviving to thriving by helping you feel in control of your health and wellbeing by addressing mental health, general health, wellbeing and inclusion needs.

This will be done using a variety of techniques including:

  • Providing individual person centred support to explore ways of improving your mental health and well-being.
  • Support you to utilise self-help facilitated tools to aid individual goal planning and measurement of your recovery.
  • Support you to make informed choices about your health and wellbeing, which may include improving your levels of physical activity, promoting a healthier diet and improving mental wellbeing.
  • Provide support with access to wider community activities and support as part of an individual plan tailored to suit you.

We know many people are struggling for different reasons this year, redundancy, debt, illness, loss of a loved one and much more. These all impact on our wellbeing and we can support you with these concerns and help get you the right support you need as well as the confidence and belief in your own abilities.
